Open a new report file and add/delete pages as desired.

Next, add content such as comments, responses, images, etc., if desired; additional content should be added prior to saving the file as a template. For example you can affix a signature and add license number in the certification page.

With the desired pages for the template arranged in the preferred sequence in the Forms Manager, click the File Tab…

and then click Save As…Save As Template.

A directory window will open to allow you to name and same the new template in the Templates folder.

Once saved, the custom template is accessed for starting a new report by choosing New from Template in the list under the File Tab.

The directory will display all available templates, including “My” custom templates.

Select the template and click Open.
